這是《無間道》之後再拍的第三部續集，故事描寫在陳永仁死後，劉建明陷入更多複雜的內幕調查與利害衝突之中。而電影也同時倒敘回到陳永仁當年為取得老大韓琛的信任，不惜以身試法，一次次犯下更多罪行。他漸漸迷失了自己，直至遇上心理女醫師DR.LEE，在接受治療的過程間，兩人漸生情愫。

Infernal Affairs III is set ten months after the original movie（Infernal AffairsⅠ）. Ming has to cope with the consequences of his actions and is struggling to maintain his position in the police force. It also turns out that Sam has quite a few moles in the police department that are being eliminated one by one.
